When to Refer Surrogates for a Re-Evaluation—And Why It Matters
Life changes. Support should too. In third-party reproduction, a psychological evaluation is often one of the first checkpoints in the surrogacy journey. But it shouldn’t be the last. Even the most emotionally prepared surrogate can face new challenges, emotional shifts, or evolving life circumstances—and when they do, a second evaluation can make all the difference.
At Thallo Health, we believe re-evaluations aren’t about questioning someone’s readiness. They’re about protecting everyone involved—surrogate, agency, and intended parents alike.
Here’s when—and why—it matters.
Why Re-Evaluations Are Essential in Surrogacy
Initial mental health screenings offer a snapshot of emotional readiness. But surrogacy is a long, deeply personal journey. Life doesn’t stand still—and neither should support.
A re-evaluation can help:
- Reassess emotional readiness in light of new experiences or stressors
- Address new concerns before they grow into complications
- Ensure alignment between surrogate and intended parents after time has passed
It’s not about red flags—it’s about responsible care.
When to Consider a Re-Evaluation for a Surrogate
1. A Major Life Change
New relationship? Family crisis? Relocation?
Life events can impact a surrogate’s emotional bandwidth and capacity to carry for someone else. If a surrogate experiences a major shift—divorce, illness, job change, financial stress—this is a good time to revisit their evaluation and ensure they feel fully supported.
2. Delays or Extended Gaps
If it’s been 9–12 months (or more) since the original evaluation, a lot may have changed. Delays due to agency coordination, legal processes, or matching timelines can affect a surrogate’s mental and emotional state.
A re-evaluation ensures the surrogate still feels confident and aligned with the process before a medical cycle begins.
3. New Concerns from Your Team
Sometimes your team picks up on something subtle: a change in communication, hesitation in tone, or feedback from the intended parents. Trust that instinct. A re-evaluation offers a supportive way to check in, address concerns, and re-ground the match.
What a Re-Evaluation Looks Like at Thallo Health
Re-evaluations aren’t just repeat screenings—they’re tailored check-ins designed to meet the surrogate exactly where she is, emotionally and mentally.
Our re-evaluation process includes:
- Clinician-led interview to explore any life updates, emotional concerns, or new dynamics
- Updated psychosocial and psychological testing, if needed
- Detailed recommendation report to guide next steps
- Open communication with the agency or clinic to ensure cohesive care
Our goal is always to empower—not delay.

In addition to evaluations, Thallo Health offers a Complimentary Mental Health Support Program for all surrogates we screen. This includes three free mental health sessions they can use at any point in their journey—whether during pregnancy or postpartum.
We’ve seen firsthand how these sessions reduce anxiety, increase communication, and improve long-term outcomes—for everyone involved.
